Application Number |
Display only |
Displays the original application number. The link is available to internal roles with access to the application. |
Permit Number or License Number |

Displays the original number from the permit application. The link is available to internal and external roles with access to the application and permit. |
Department Error |

The Department Error check box is available only to PennDOT receivers. It defaults to unchecked. If you believe a supplement is needed to correct an error made by PennDOT, contact your District Permit Office. |
Supplement Type |
Display only |
Displays the supplement type selected from the Create Supplement button dropdown list. In the case of a Time Extension Supplement, it displays Time Extension. |
Supplement Expiration Date |
Required |
This is the Date the new supplement being created will expire. The Supplement Expiration Date options include 6 Months, 12 Months, and Other.
A selection must be made in order to save and/or move away from this screen. |
Current Expiration Date |
Display only |
Displays the expiration date identified on the permit or most recently issued time extension supplement. |
New Expiration Date |
Display only |
Displays the expiration date for the new supplement being created. The date displayed here is dependent upon the time frame selected from the Supplement Expiration Date field (see Above). If Other is selected from the Supplement Expiration Date field, there is no New Expiration Date displayed. |
Applicants Supplement Description and Justification |

The user must enter text into this field in order to save and/or move away from this screen. The text should include a reason why the Time Extension is needed. |
Supplement Type |
Display only |
Displays the supplement type selected from the Create Supplement button dropdown list. In the case of a Change in Work Supplement, it displays Change in Work. |
Applicants Supplement Description and Justification |

The user must enter text into this field in order to save and/or move away from this screen. The text should include a description of how the work is changing and why it is changing. Document changes to work details on the Minimum Use Driveway (MUD) form (M-950A) in this field. The MUD fields are not available on a Change in Work supplement. |
Supplement Type |

Displays the supplement type selected from the Create Supplement button dropdown list. In the case of a Fee Change Supplement, it displays Fee Change. |
Fee Change Reason |

The user must enter text into this field in order to save and/or move away from this screen. The text should indicate why a fee change supplement is necessary. |

Displays the supplement type selected from the Create Supplement button dropdown list. In the case of a Permittee Name Change Supplement, it displays Permittee Name Change. |
