The recording notice Email that is sent to the applicant team shows who the application was submitted by, their contact information, and permit specific information such as location of work and type of permit. It also contains three attachments, the recording document (M945-RC), the Conditionally Approved permit, and the Application Response Letter.

Your application for Highway Occupancy Permit has been conditionally approved. However, prior to the issuance of your Permit, the Department must record this Permit in the County Recorder of Deeds Office, so that future purchasers of the affected property will be given notice of the restrictions contained in the Permit as well as their duty to maintain the occupancy authorized in the Permit.
In order for the Permit to be recorded, it will be necessary for you to complete and sign Block "A", and notarize Block "B" on the document "Recording Copy" form (M-945 RC). Also add the Deed Book Volume and Page of your property in Block "A".
Please return the original signed and notarized form to the Engineering District Office with a check in the amount of $25.00 to cover the cost of recording the Permit. Please make your check payable to the "Recorder of Deeds".
You will receive your Highway Occupancy Permit when the conditionally approved Permit copy is recorded, so please return it signed and notarized with a check within 30 days. If the Department does not receive your response within 30 days, the Department will deny your application.
