Dropdown indicating the type of insurance (Commercial Insurance, Self Insurance, Both). When a customer account is activated, it must have insurance coverage in force.

Insurance Confirmation Date

Calendar field used to select the date insurance information is confirmed. Must be greater or equal to the current date.

PennDOT Procedure: Account Managers are required to confirm self-insured coverage by April 30 each year. When the type of coverage is Both, the Account Manager enters the earliest date insurance must be reconfirmed.
